Introducción. La enfermedad de Huntington (EH) es un trastorno neurodegenerativo autosómico dominante caracterizado por disfunción motora, deterioro cognitivo y trastornos psiquiátricos. El uso de dispositivos wearables ha aumentado notablemente, tanto en la vida cotidiana como en la investigación clínica, ofreciendo nuevas oportunidades para monitorizar de forma continua y objetiva parámetros relacionados con la salud.&#13;
Objetivo: Evaluar la utilidad de dispositivos wearables de muñeca (Fitbit) en pacientes con EH, con el fin de caracterizar de manera objetiva los patrones de actividad física y sueño en esta población, y contrastarlas con la percepción reportada por los pacientes.&#13;
Metodología. Se realizó un estudio observacional longitudinal de un año con pacientes sintomáticos y ambulatorios con EH. La actividad física y el sueño se registraron de forma objetiva mediante dispositivos Fitbit, mientras que la percepción subjetiva del paciente sobre estos aspectos se evaluó mediante el cuestionario IPAQ y el cuestionario específico de sueño HD-SLEEP. Además, se midieron parámetros clínicos y de sarcopenia tanto al inicio como a los 12 meses. Posteriormente, los participantes se clasificaron en dos clústeres en función de la edad, la función motora y la función cognitiva.&#13;
Resultados. Se incluyeron 34 sujetos con EH, con una edad mediana (IQR) de 54 años (40;60) años, un número de repeticiones CAG de 43 (41;45) y, de las cuales, el 53% eran mujeres. El grupo de pacientes con mejores características clínicas (Clúster 1) mostró mejor calidad del sueño y menor presencia de sarcopenia. Aunque al inicio no se observaron diferencias claras en los niveles de actividad física, al cabo de un año este clúster presentó una tendencia a la disminución de dicha actividad. Los pacientes con menor fuerza muscular tenían un sueño de peor calidad; y, aquellos con peor calidad del sueño caminaron menos pasos por día. La concordancia entre Fitbit e IPAQ fue baja.&#13;
Conclusiones. La disminución de la actividad física podría considerarse un indicador temprano de progresión de la enfermedad, además, la autopercepción de la actividad capta dimensiones diferentes a las registradas de forma objetiva. Un estado clínico más avanzado se asoció con una peor calidad del sueño y mayor sarcopenia. Los dispositivos Fitbit se presentan como herramientas útiles para la monitorización continua de personas con EH y para apoyar decisiones clínicas tempranas.; Introduction. Huntington’s disease (HD) is an autosomal dominant neurodegenerative disorder characterized by motor dysfunction, cognitive impairment, and psychiatric disturbances. The use of wearable devices has increased notably both in daily life and in clinical research, offering new opportunities to continuously and objectively monitor health‑related parameters. Objective. To evaluate the usefulness of wrist‑worn wearable devices (Fitbit) in patients with HD, with the aim of objectively characterizing physical activity and sleep patterns in this population, and to contrast these measures with the patients’ self-reported perceptions. Methodology. A one‑year longitudinal observational study was conducted with symptomatic and ambulatory patients with HD. Physical activity and sleep were objectively recorded using Fitbit devices, while the patients’ subjective perception of these aspects was assessed through the IPAQ questionnaire and a specific sleep questionnaire. In addition, clinical and sarcopenia parameters were measured both at baseline and after 12 months. Participants were subsequently classified into two clusters based on age, motor function, and cognitive function. Results. A total of 34 subjects with HD were included, with a median age (IQR) of 54 years (40;60), a CAG repeat length of 43 (41–45), and of whom 53% were women. The group of patients with better clinical characteristics (Cluster 1) showed better sleep quality and a lower presence of sarcopenia. Although no clear differences were observed in physical activity levels at baseline, after one year this cluster showed a tendency toward decreased activity. Patients with lower muscle strength had poorer sleep quality, and those with poorer sleep quality walked fewer steps per day. Agreement between Fitbit and IPAQ was low. Conclusions. The decline in physical activity may be considered an early indicator of disease progression; moreover, self‑perceived activity captures dimensions different from those recorded objectively. A more advanced clinical stage of the disease was associated with poorer sleep quality and greater sarcopenia. La presente tesis analiza la ansiedad ante los exámenes en estudiantes universitarios&#13;
mediante dos investigaciones complementarias. La primera evalúa la eficacia de un&#13;
programa de control de ansiedad en dos versiones: una extensa (CAE Largo) y su&#13;
abreviación (CAE Breve). Los resultados muestran que ambas intervenciones reducen&#13;
significativamente la ansiedad general y la ansiedad ante exámenes, sin diferencias&#13;
relevantes en eficacia o satisfacción, destacando especialmente los componentes&#13;
cognitivos. La segunda investigación compara tres modalidades de desensibilización&#13;
sistemática para la reducción de la ansiedad ante exámenes: vídeos 360º, realidad&#13;
virtual e imaginación guiada. Aunque las tres reducen la ansiedad, los vídeos 360º&#13;
obtuvieron mejores resultados en eficacia, satisfacción y sensación de presencia. En&#13;
conjunto, la tesis concluye que una intervención breve con componentes esenciales&#13;
puede ser suficiente y que los vídeos 360º representan una alternativa terapéutica&#13;
eficaz, accesible y bien aceptada para tratar la ansiedad ante los exámenes; This thesis examines test anxiety among university students through two complementary&#13;
studies. The first study evaluates the effectiveness of an anxiety management program&#13;
in two formats: a full-length version (Long CAE) and its abbreviated version (Brief CAE).&#13;
The results indicate that both interventions significantly reduce general anxiety and test&#13;
anxiety, with no relevant differences in effectiveness or satisfaction, highlighting&#13;
particularly the contribution of cognitive components. The second study compares three&#13;
systematic desensitization modalities for reducing test anxiety: 360º videos, virtual&#13;
reality, and guided imagery. Although all three modalities reduced anxiety levels, 360º&#13;
videos yielded superior outcomes in terms of effectiveness, satisfaction, and sense of&#13;
presence. Overall, the thesis concludes that a brief intervention preserving the essential&#13;
components of the original program may be sufficient and that 360º videos constitute an&#13;
effective, accessible, and well-accepted therapeutic alternative for the treatment of test&#13;
anxiety

La proteína NS1 del virus de la gripe A desempeña múltiples funciones províricas en células infectadas. Esta proteína multifuncional interactúa con numerosos factores celulares, algunos de relevancia biológica aún poco definida, como su capacidad para unirse y activar las PI3K de clase IA. Las PI3K son quinasas lipídicas reguladas que actúan como nodos clave en redes de señalización celular. La activación de PI3K por NS1 se produce mediante su unión a la subunidad p85β, aunque la importancia de esta especificidad y la contribución de las isoformas de p110 siguen siendo desconocidas. Para estudiarlo, hemos desarrollado un ensayo BiFC que permite seguir heterodímeros específicos de PI3K. Nuestros resultados muestran que NS1 induce una relocalización y activación dependientes de la isoforma de los heterodímeros de PI3K. Además, mutaciones oncogénicas en ambas subunidades imitan el efecto de NS1 y rescatan parcialmente la pérdida de función en virus mutantes; The non-structural protein 1 (NS1) of influenza A virus performs a broad variety of proviral activities in the infected cell, primarily mediating evasion from the host innate immune response by being the main viral interferon antagonist. However, among the multiple interactions described for this small multifunctional protein, there are several whose biological relevance remains obscure, such as the ability of NS1 to bind and activate class IA phosphoinositide 3kinases (PI3Ks). PI3Ks are highly regulated lipid kinases that act as critical nodes in multiple cell signaling networks and regulate cellular physiology, including differentiation, growth, survival, trafficking, and immune function. As such, PI3Ks are also important proto-oncogenes whose deregulation lies behind a substantial number of different human cancers. Structurally, class IA PI3Ks are heterodimers formed by a regulatory (p85) and catalytic (p110) subunit, of which there are several isoforms described, adding further layers of complexity to their activity. Activation of PI3K by NS1 is mediated by NS1 binding to the p85 subunit. Interestingly, such binding is specific to the p85β isoform, and PI3K heterodimers containing p85β and any of the other three p110 isoforms are presumably activated by NS1. However, the significance of this p85β specificity, and the contributions of different p110 isoforms, remains unknown.   In order to better understand the consequences of PI3K activation by NS1, we have developed a bimolecular fluorescence complementation (BiFC) assay to selectively track the different PI3K heterodimers according to their specific regulatory and catalytic isoforms, as well as to assess their behavior upon activation. Using this system, we found that NS1 induces an isoform-specific relocation and activation of the different PI3K heterodimers. However, the effects of other known activators of PI3K, such as Ras and Src, were different from those induced by NS1. We found that clinically relevant, oncogenic hyper-activating mutations in both catalytic and regulatory subunits of PI3K could mimic the effect caused by NS1, and partially rescued the loss of viral fitness in a recombinant virus encoding a p85β-binding deficient NS1. We postulate that by mimicking an oncogenic deregulation of the PI3K pathway, influenza A virus induces a transient, transformed-like status in the infected cell to stimulate virus replication

La tesis examina el abordaje rehabilitador de la parálisis facial periférica en un hospital de tercer nivel, centrado en la caracterización funcional, clínica y sociodemográfica de los pacientes remitidos al Servicio de Rehabilitación. Mediante un diseño observacional, se analizan parámetros clave para la planificación terapéutica, como el grado inicial de disfunción motora facial, la etiología, la lateralidad, el tiempo de evolución y la especialidad derivadora. Se emplean escalas estandarizadas de valoración facial para cuantificar la afectación y monitorizar la respuesta al tratamiento. Asimismo, se estudian los factores clínicos asociados a la indicación de toxina botulínica como herramienta de modulación neuromuscular en secuelas crónicas, así como su relación con la afectación funcional y psicológica. Los resultados permiten identificar perfiles rehabilitadores, optimizar la selección de intervenciones y reforzar criterios basados en evidencia para una gestión multidisciplinar eficiente de la parálisis facial periférica; This thesis examines the rehabilitative management of peripheral facial paralysis in a tertiary-level hospital, focusing on the functional, clinical, and sociodemographic characterization of patients referred to the Rehabilitation Service. Using an observational design, the study analyzes key parameters for therapeutic planning, including the initial degree of facial motor dysfunction, etiology, laterality, time since onset, and referring specialty. Standardized facial assessment scales are employed to quantify impairment and monitor treatment response. The research also evaluates clinical factors associated with the indication of botulinum toxin as a neuromuscular modulation tool in chronic sequelae, as well as its relationship with functional and psychological impact. The findings allow the identification of rehabilitative profiles, optimization of intervention selection, and reinforcement of evidence-based criteria for efficient multidisciplinary management of peripheral facial paralysis
