El cáncer de pulmón continúa siendo una de las principales causas de mortalidad a nivel
mundial, destacando la necesidad de estudios que exploren tanto su evolución como los factores
que influyen en su diagnóstico y tratamiento. Esta tesis doctoral analiza los factores predictivos
del cáncer de pulmón en la región de Burgos a lo largo de un período de 20 años (2000-2021),
evaluando el impacto de la pandemia de COVID-19 en el diagnóstico y la supervivencia de los
pacientes, así como las características clínicas y tomográficas de las mutaciones en EGFR y
ALK en casos de adenocarcinoma de pulmón.
En el primer eje del estudio, se realiza un análisis longitudinal de la incidencia, mortalidad
y supervivencia del cáncer de pulmón, revelando tendencias epidemiológicas específicas de la
región. Los resultados destacan una clara variación en la prevalencia de tipos de cáncer de
pulmón entre géneros, con una mayor incidencia de adenocarcinoma en mujeres y de carcinoma
epidermoide en hombres. Este análisis proporciona información valiosa para optimizar las
políticas de salud pública y los programas de cribado en la región.
El segundo eje aborda los efectos de la pandemia de COVID-19, identificando una
disminución en la supervivencia y una alteración en el diagnóstico temprano durante los años
críticos de la pandemia. Los pacientes diagnosticados durante la pandemia presentaron un
menor tiempo de supervivencia en comparación con aquellos diagnosticados previamente,
subrayando la necesidad de desarrollar estrategias resilientes para mantener la atención
oncológica en tiempos de crisis.
El tercer eje del estudio examina las mutaciones en EGFR y ALK en adenocarcinomas de
pulmón, resaltando su relevancia para la personalización de los tratamientos. Los pacientes con
mutación en EGFR mostraron una supervivencia significativamente mayor que aquellos sin
alteraciones genéticas, lo que refuerza la importancia de identificar estas mutaciones para
optimizar las terapias dirigidas. Además, se evidenció una mayor prevalencia de mutaciones en
pacientes no fumadores, y una correlación significativa entre características tomográficas y
mutaciones genéticas.
Los resultados de esta investigación tienen implicaciones directas para la práctica clínica y
las políticas de salud, proponiendo mejoras en la detección temprana y tratamiento del cáncer
de pulmón, así como en la adaptación de los sistemas de salud ante emergencias globales.
Asimismo, subrayan la importancia de continuar investigando sobre los factores genéticos y
clínicos asociados al cáncer de pulmón para mejorar el pronóstico de los pacientes.
Lung cancer remains one of the leading causes of mortality worldwide, underscoring the

importance of identifying these mutations to optimize targeted therapies. Additionally, a higher
prevalence of mutations was found in non-smoking patients, and a significant correlation was
observed between tomographic characteristics and genetic mutations.
The findings of this research have direct implications for clinical practice and health policies,
proposing improvements in early detection and treatment of lung cancer, as well as in adapting
health systems during global emergencies. Furthermore, the study underscores the importance
of continued research into the genetic and clinical factors associated with lung cancer to
improve patient prognosis.
This multidimensional study significantly contributes to the understanding of lung cancer,
offering new perspectives on its management and prevention, and opening future lines of
research that could be extended to other regions and healthcare contexts.
